School Age Care Child’s Record Form – (Pack of 12)

Under the Child Care Act 1991 (Early Years Services) (Registration of School Age Services) Regulations 2018, our School Age Care service is required to have specific information on your child, their family and emergency contacts. Each child is unique, and it is important we get to know your child, and their interests as this information will help with the settling in process and the care of your child.

The School Age Care Child’s Record Form records all the details of the child attending the service, including:

• Personal details
• School collection arrangements
• Authorisation to collect details
• Emergency contact details
• Medical information
• Getting to know your child
• Consent for emergency medical treatment
• Consent for temperature reducing medication
• Consent for sun cream

The Child Care Act 1991 (Early Years Services) (Registration of School Age Services) Regulations 2018 requires parental/guardian consent for appropriate medical treatment should the need arise.
