Return to Work Form

Return to Work Form – Use this resource to record return to work meetings with staff members after periods of absence due to illness.  Record details of the absence in the first section. The next section outlines the staff members record of absence. The final section details the return to work and any follow up actions required.

It will assist you in keeping communication open and frequent with staff members regarding absence. It will assist in supporting the employees return to work whilst also enabling managers to manage absences in their service. Use this resource to allow for accurate recording of all meetings and make tracking absence trends easier.

This resource falls under employment legislation and workplace policies. It helps to identify any reasonable accommodations and training and/support requirements.

Under Section 10 of the Safety, Health and Welfare at Work Act 2005, employers must provide their employees with:

• the necessary instruction
• training
• supervision on health and safety

Thie Return to Work Form is A4 size and landscape orientation. Each page has a duplicate so that one copy can be given to the staff member and one copy can be placed on their file.
