The nappy changing log is used to record a child’s nappy changes each day. There is space to include if it was normal or not and an area to add in comments.

Notes:

Pallor/ Skin colour – Observe the colour of the face. If the colour is abnormal, take action.
Breathing – Observe and listen to the child’s breathing. If the breathing is abnormal, take action.
Sleep position – Position the child on their back with their head uncovered. Place feet at the foot of the cot. Reposition the child if found sleeping on their front.
Sleep room – Ensure the temperature of the sleep room is between 16 – 20°C
If you note abnormal pallor, breathing, or position, take action, and follow the policy of the service.
