Child’s Record Form (11-Pack)

Early Childhood Ireland’s Child’s Record Form records all the details of the child attending the service and comes in a pack of 11 forms. Crucially, you can easily record and access important medical information on this form. This A4 form allows you to record a variety of important administrative details for a child in your setting, such as:

• authorisation to collect details.
• consent for emergency medical treatment
• emergency contact details and medical information (including dates and details of immunisations)
• consent for temperature reducing medication
• getting to know your child
• and other relevant consents* such as for sun cream.

Under the Early Years Services Regulations (2016), an Early Years service is required to have specific information on the children attending the service, their families and emergency contacts. Each child is unique, and it is important to get to know the children, and their interests. This information will help with the settling in process and the care of the children.

What about School Age Care settings?

Early Childhood Ireland has also developed a School Age Care Child’s Record Form, allowing School Age Care settings to record all pertinent information required under the Child Care Act 1991 (Early Years Services)(Registration of School Age Services) Regulations 2018.
