Early Childhood Ireland’s Accident and Incident Book – Under the Early Years Services Regulations (2016), Early Learning and Care or School Aged Childcare services are required to record any accident, injury or incident involving a child attending the service (Regulation 16 Record in relation to a preschool service).
Fill out the Accident and Incident Form using a pen and make sure all sections are fully completed. The person with responsibility for completing this form must include all relevant details of the accident or incident, such as:
- Location
- Time and date the accident/incident that occurred
- Details of the ratios at the time of the accident/incident
- Details of any injuries
- Details of staff present
- Any action taken to correct the situation and prevent reoccurrence
The staff member in charge of completing the form must sign all forms and get the parent/guardian to sign the form. Identify the children in the form by using their full name, rather than just a shorthand version.
Document any care given in a first aid situation in line with the service’s policy on accidents and incidents (Regulation 23 Safeguarding Health, Safety and Welfare of Child).

The Accident and Incident Book is a handy way to fill out reports that comply with the regulations. The forms in the Accident and Incident book are in triplicate format; so keep one form in the book; one copy should be given to the parent/guardian, and store one copy in the child’s file in the service.
