New Monthly Awards Application Process

As a result of the General Data Protection Regulation (GDPR) coming into place on 25th May 2018, we are amending the way we receive applications for our Learning Story and Inspired Practice monthly awards.

As before, applications are to be emailed in to us, however we will now need a copy of the consent form for every child or adult that appears in the learning story or inspired practice application. These can either be scanned/photographed and emailed in with your application, or they can be posted into us (please note, your application will not be included until we receive all consent forms).

 

As well as this, you will also now need to complete a Consent Submission Form. In this form, the names of each person appearing in the photographs in the application (child or adult) are to be listed, along with the number or brief description of each photograph they appear in. For example, photographs can be numbered as per the examples below, starting at 1 and continuing through the document.

This is to be done so we can allocate each consent form to the right person.

Inspired Practice Consent Submission Form

 

Learning Story and Inspired Practice Awards applications will not be accepted until we receive the necessary consent forms and the Consent Submission Form. To be included for that month’s award, applications are to be received by the 28th of each month. If we get in an application without consent forms and / or a completed Consent Submission Form, we will delete the application from our system on the last day of the month. These applications can of course be resubmitted the following month, as long as they have the relevant consent forms / Consent Submission Form.
